Primary Care Clinic for Medically Complex Children
The Primary Care Clinic is primary care for children with complex medical needs. Many of the children that we care for have complicated medication regimens, are dependent on technology (feeding pumps, ventilators, etc.), have developmental challenges, feeding issues and/or other chronic medical conditions.
We will see your child for well visits per the American Academy of Pediatrics guidelines, sick visits, and follow ups at least every three months to ensure he/she receives the coordinated care to help in being as healthy and happy as possible. During each visit, a member from behavioral health is present to provide both you and your child support. They will also conduct screenings for development, behavioral and mental health needs.
As well as being your child's pediatrician, our team will work closely with your child's healthcare specialists serving as a care coordinator for your child's multiple medical providers.
We know that home is not a hospital so we will work with you and your family to create a home schedule that makes life feel as routine as possible. We will provide you with a nurse care coordinator who will help assist with your needs that come with caring for a child with complex medical needs such as coordinating appointments, medication and equipment help, and school forms.
Who We Serve
We see children from infancy through 21 years of age who fit the medically complex criteria for the clinic. We will see older children on a case-by-case basis.
We see children with:
- Autism (along with other medical complexities)
- Cerebral Palsy
- Gastrostomy tubes
- Genetic Conditions
- Head trauma or other Neurological conditions
- Tracheostomy and/or Ventilator dependency
- Medical and psychological issues
- Prematurity with complicated nursery courses
- Complex cardiac conditions
- Seizure disorders
