To obtain a copy of medical records, please download the applicable form:

• Medical records released to patients/parents/legal guardians: Request for Medical Records Patient or Personal Representative.
• Medical records released to a third party: Authorization to Release Protected Health Information.

Return the signed authorization form by fax, mail or in person.

A photo ID is required when picking up records. Please note that only a patient or patient representative may pick up records, unless specified in writing.
